Tele ICU not a substitute for in house ICU physician and accreditation rules

April 6, 2026Nick Mark MD

Nick Mark argues tele-ICU provides backup but cannot replace an on-site intensivist for emergencies, and questions why accreditation rules do not require an ICU doctor physically in-house.

Tele-ICU is a great way to have backup on a busy shift but it’s not a replacement for having an ICU doc on site ready to respond in an emergency.
In 2026, with all the stupid JCOH rules for accreditation, I’m surprised having an ICU doc physically in house isn’t required.
